Overview
We are seeking a dedicated and experienced Registered Nurse with a strong background in home care to join our dynamic healthcare team. This vital role involves providing comprehensive nursing care directly in patients' homes, ensuring personalized treatment plans, and promoting health and well-being. Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs.
- Administer medications, injections, IV infusions, and manage catheter care while adhering to strict medical documentation and HIPAA compliance standards.
- Develop and implement personalized care plans, including dementia care, geriatrics, pediatric care, and behavioral health interventions.
- Provide specialized nursing procedures such as tube feeding, ventilator management, wound suturing, phlebotomy, and sterile processing when necessary.
- Coordinate discharge planning and case management activities to ensure seamless transitions of care across settings while complying with long-term care regulations.
- Utilize electronic health record (EHR) systems like Cerner or Epic for accurate documentation of patient care activities and medical records management.
- Supervise and collaborate with team members, including nurse management and other healthcare professionals, to ensure optimal patient outcomes and adherence to nursing policies.
Requirements
- Valid RN license with current state registration; additional certifications such as Basic Life Support (BLS), IV insertion, and medication administration are required.
- Proven home care experience with familiarity in managing complex cases involving long-term care, acute pain management, or chronic conditions such as dementia or developmental disabilities.
- Extensive knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques.
- Experience working with EMR/EHR systems like Athenahealth or eClinicalWorks; proficiency in medical documentation is essential.
- Strong clinical skills in patient assessment, vital signs monitoring, medication administration, and physical examinations.
- Ability to work independently in patients’ homes while maintaining excellent communication skills within multidisciplinary teams.
- Knowledge of state healthcare regulations, HIPAA compliance standards, infection control practices, and nursing policy adherence.
- Prior experience in diverse settings such as hospital medicine, ICU or trauma centers (Level I & II Trauma Center experience), emergency medicine, or long-term care facilities is highly desirable.

Pay: $36.95 - $44.50 per hour
