Description of job:
SnapCare is seeking a skilled and patient‑focused Registered Nurse (RN) to provide PRN coverage in a Skilled Nursing Facility (SNF) setting at Spring Lake Village Care Center in Santa Rosa, CA. This role involves coordinating resident care, supervising LPN/LVNs and CNAs, and ensuring all care is delivered in compliance with state and federal regulations.
The RN will support patient outcomes by managing care plans, administering treatments, and maintaining a safe, collaborative care environment.
Qualifications:
- Current California RN license in good standing
- Minimum 1 year of RN experience
- Experience in skilled nursing, rehabilitation, or long‑term care preferred
- Strong knowledge of care planning and nursing supervision
- Ability to recognize and respond to emergencies
- Strong documentation and organizational skills
Responsibilities:
- Coordinate, supervise, and direct CNA and LVN staff
- Perform patient assessments and develop individualized care plans
- Administer medications and treatments per physician orders
- Monitor resident conditions and communicate changes to providers
- Ensure compliance with federal, state, and facility regulations
- Maintain accurate and timely documentation of all care provided
- Assist with admissions, evaluations, and referral reviews when needed
- Promote patient safety and ensure resident rights are protected
- Maintain inventory of supplies and support efficient unit operations
- Foster a safe, clean, and supportive care environment
