Home Health Registered Nurse (RN) – Home Health
Location: St Johnsbury VT
Shift: M-F
Pay Rate: $71/Hourly
Mandatory Qualifications for this Job
- Active Vermont RN License (or Compact License)
- ADN or BSN from an accredited nursing program
- Current BLS/CPR Certification
- 1–2 years of Acute Care and/or Home Health RN experience
- OASIS documentation experience required
- Skilled in patient assessment, care planning, medication administration, wound care, and case management
- Strong clinical judgment, communication, and documentation skills
- Valid Driver's License, insured vehicle, and ability to travel to patient homes
- Ability to lift up to 50 lbs and perform essential patient care duties
Roles and Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans
- Provide skilled home health nursing, including medication administration, treatments, and wound care
- Complete OASIS assessments, clinical documentation, and patient progress notes
- Monitor patient conditions and communicate changes to physicians
- Educate patients and families on disease management, medications, and home safety
- Coordinate care with physicians and the interdisciplinary healthcare team
- Serve as Case Manager and support discharge planning and continuity of care
- Ensure compliance with nursing standards, infection control, and patient safety while delivering compassionate, patient-centered care
