Full-Time Physical Therapist (PT) – Split Opportunity
Locations: Aventura at Carriage Inn & Aventura at Shiloh Springs
Location: Dayton &Trotwood, OH
Setting: Skilled Nursing / Long-Term Care (SNF/LTC)
Aventura at Carriage Inn and Aventura at Shiloh Springs are seeking a Full-Time Physical Therapist (PT) to join our energetic and supportive rehab team. This split opportunity offers the chance to work across two nearby facilities while making a meaningful impact on the lives of residents every day.

Responsibilities:
Evaluate and treat residents in a SNF/LTC setting
Develop and implement individualized therapy treatment plans
Help residents improve mobility, strength, balance, and overall function
Collaborate with nursing, therapy, and interdisciplinary teams
Complete documentation using Net Health and PointClickCare (PCC)
Maintain compliance with facility and regulatory standards
Qualifications:
Licensed Physical Therapist (PT) in the state of Ohio
SNF/LTC experience preferred, but not required
New graduates are welcome to apply
Strong communication and clinical skills
Positive attitude and team-oriented mindset
