Grace Community Home Health is a Christian/Faith-Based Agency seeking a Per Diem Occupational Therapist who has a desire to provide excellent care, and wants to be part of a team who loves to serve our community.
Occupational Therapist Home Health Position Summary
- Full independence and schedule flexibility
- Independently evaluate and conduct follow-up visits as an Occupational Therapist
- Provide Occupational Therapy services to clients in their home
- Document patient assessment and visit details in EHR and notify Clinical Supervisor of findings
- Communicate care and updates with the care team
- Submit documentation of Evaluation and Follow-up visits according to the company policies
- Evaluate and create plan of care for patients independently
Qualifications
- Friendly, cheerful, and excellent customer service/bedside manners
- Minimum of 1-year experience seeing patients in the home setting
- Active California Occupational Therapy License
This is a Per Diem position. Annual Full Time Salary equivalent $143k, depending on experience.
