Responsibilities
- Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and transfer methods.
- Monitor and record vital signs accurately, including blood pressure, temperature, pulse, and respiration rates, maintaining detailed medical documentation in electronic health records (EHR) systems.
- Support medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations to maintain patient confidentiality.
- Provide emotional support and companionship to patients and their families, fostering a comforting environment during difficult times.
- Collaborate with interdisciplinary teams to coordinate patient care plans, including hospice-specific needs such as Alzheimer's care, dementia care, memory care, and behavioral health considerations.
- Follow aseptic techniques and infection control protocols diligently to prevent the spread of infectious diseases within the care setting.
- Assist with patient transfers using safe lifting practices and equipment like Hoyer lifts or patient transfer methods suitable for geriatric or disabled patients.
Experience
- Prior experience working in hospice care or with terminally ill patients is highly preferred.
- Demonstrated competence in patient service roles within long-term care facilities, nursing homes, assisted living communities, or home care settings.
- Knowledge of medical terminology related to geriatrics, palliative care, and chronic illnesses such as Alzheimer's disease or other dementias.
- Experience working with electronic health records (EHR/EHR systems) like Epic or Cerner is advantageous.
- Critical care experience or ICU background enhances your ability to manage complex patient needs effectively.
- Familiarity with medical-surgical procedures, medication administration, phlebotomy, and basic life support (BLS) certification are valuable assets.
- Ability to handle manual lifting safely and perform vital signs measurement accurately while maintaining patient confidentiality per HIPAA standards.
