In this role, you will provide high-quality, patient-centered physical therapy services in the comfort of patients' homes, helping them improve mobility, manage pain, and regain independence.
- Deliver physical therapy treatments to patients in their homes according to prescribed plans of care.
- Evaluate patient progress and adjust treatment plans as necessary.
- Maintain accurate and timely documentation of patient care and treatment outcomes.
- Communicate effectively with patients, families, and healthcare team members.
- Ensure compliance with all company policies, professional standards, and regulatory requirements.
- Valid license as a Physical Therapist.
- Previous homecare experience preferred.
- Strong assessment and clinical skills.
- Excellent communication and interpersonal skills.
- Reliable transportation and ability to travel within the Southeast MI Regional area.
- Competitive pay: $70.00 - $80.00 per visit, paid bi-weekly.
