We are seeking a Physical Therapist for a high-volume skilled nursing and rehabilitation center in San Rafael, CA. This facility offers short-term rehab, long-term care, orthopedic recovery, and therapy for medically complex residents.
You will conduct thorough evaluations, create individualized treatment plans, and deliver functional and mobility-based therapy interventions. This center offers strong interdisciplinary support, a busy caseload, and slightly higher guaranteed hours.
Perform PT evaluations and assessments.
Create individualized care plans tailored to patient goals.
Deliver therapeutic exercises, gait training, and functional mobility activities.
Modify treatment plans based on clinical progress.
Document evaluations, treatments, and progress notes accurately.
Collaborate closely with nursing, physicians, and therapy colleagues.
Provide caregiver and patient education.
Maintain compliance with regulatory requirements.
Valid California PT license.
SNF experience preferred but not required.
Strong interpersonal, clinical, and documentation skills.

Higher guaranteed weekly hours (38 hours).
