Job Purpose:
Under the general supervision of the Public Health Nurse / Public Health Manager, the Community Health Representative II (CHR II) directly impacts individuals and the community by working with health care providers and the community to provide basic healthcare, and health education on chronic disease within their scope of practice and license. As a member of the Health Care Team, he/she is also responsible for liaising between patients, families, the community, a health care provider to ensure patients and families understand their health conditions and treatment and are receiving appropriate care. The CHR II will also monitor the home and community to identify the required healthcare resources, and public health interventions that may be needed.
High school diploma or general education degree (GED). Certified Medical Assistant. (CMA)
Minimum of two (2) years' experience preferred working in a clinical setting, chronic disease outreach, and/or community-based outreach program.
Completion of web based IHS training for CHR Online Training Modules or a certificate of completion of the CHR Certificate Program within 6 months of hire.
Maintain patient confidentiality. Ability to work well within a multidisciplinary team.
Possess current CPR and First Aid certification or be willing to obtain within three (3) months of hire.
Must have good communication skills, both Written and verbal.
Make home visits routinely to Native American patients that reside in the Sonoma County area.
Must possess a current California Driver’s License and be insurable.
Must pass a pre-employment drug screen, physical and background investigation
Must occasionally lift and/or move up to 50 pounds
Special Qualifications: All employees must be sensitive to the needs of the Native American community, demonstrate a deep understanding and respect for the diverse cultural heritage, traditions, and historical context of the Native American community.
