Full Job Description
As a PRN or part‑time LPN or RN, you will provide skilled nursing, chronic care management, and routine home care for a medically complex patient. Care needs include general hands‑on support.
Shifts are minimum of 4 hours, with flexible scheduling options.
Responsibilities
- Provide direct skilled nursing care including trach, PEG, wound care, and chronic condition management
- Complete client assessments, routine evaluations, and documentation
- Maintain accurate and timely documentation for all encounters
- Perform light housekeeping tasks related to patient care (tidying, laundry, simple meal prep)
- Maintain compliance with company policies, state regulations, and professional standards
- RN may conduct PCA/CNA training and skills check‑offs
Requirements
- Diploma, Associate, or Bachelor’s degree in Nursing
- Current LPN or RN licensure
- Valid driver’s license and acceptable MVR
- Clinical experience preferred but new graduates with strong skills are welcome
- Ability to work independently in a home environment
- Strong documentation and communication skills
