BAYADA Home Health Care is seeking an experienced and compassionate RN Nurse Case/Clinical Manager to join our Adult Nursing Home Care office in Plymouth, Minnesota. This is an exciting opportunity for a Registered Nurse who is passionate about leadership, mentorship, and delivering care with the highest professional, ethical, and safety standards.

Location: 3033 Campus Drive, Suite E280, Plymouth, MN 55441
Schedule: Monday through Friday, 8:30 AM to 5:00 PM
Pay Range: $85,000 to $95,000 annually, based on experience
The RN Nurse Case/Clinical Manager serves as a key clinical leader within the office, supporting both client care and staff development. This role blends hands-on clinical involvement with office-based leadership responsibilities. The ideal candidate is a strong nurse leader who can guide field staff, support quality outcomes, and contribute to a positive, mission-driven team culture.
Provide education and ongoing clinical support to staff, including oversight of a nurse residency program
Lead and support field clinicians, including RNs and LPNs, in delivering exceptional client service and high-quality clinical care
Ensure adherence to BAYADA policies, procedures, and clinical standards
Visit clients in their homes to supervise field clinicians and support quality care delivery
Maintain accountability for internal case management activities
Evaluate, mentor, and develop field staff to support performance and professional growth
Assist with in-home client assessments and the care planning process by providing clinical guidance to the team
Partner closely with both clients and office staff to ensure coordinated, responsive care
Monitor clinical practice, quality assurance, and case oversight across the office
Current Registered Nurse (RN) license in good standing in the state of Minnesota
Minimum of three years of recent, verifiable hands-on experience as an RN or LPN, preferably in home care
At least one year of experience in nursing management or pediatric nursing
BSN or MSN preferred
Child Abuse Clearance, where applicable
Trach, vent, and tube experience within a home health care setting preferred, along with the ability to teach and train other clinicians
Strong clinical assessment and organizational skills
Previous supervisory experience with field staff preferred
Experience with clinical oversight, quality assurance, and case management preferred
Demonstrated ability to work independently while taking on increased or diverse responsibilities
Ambition to grow and advance beyond the current role
Strong communication and PC skills
