Then the South Carolina Department of Social Services has the right job opportunity for you.Job Duties: The Adult Centered Planning (ACP) Coordinator provides an array of engagement services, as assigned, for a specific region of the state. This role will be key in the development and implementation of the Agency's ACP practice region wide, utilizing supportive decision making techniques and a person centered approach. The ACP Coordinator will cover surrounding counties in the Upstate region.Provides an array of Adult Advocacy Engagement and Planning services through preparation, planning, and facilitation of adult centered planning meetings (ACPMs) in assigned region and provides coverage as needed to other regions.Identifies/assess the need for adult centered planning meetings, develops and monitors ACPM referral process, and holds pre-meetings with adults to prepare them for the ACPMs.Documents the ACPMs accurately in CAPSS.Supports the data collection and tracking of ACPMs in assigned region and participates in outcome reporting processes to ensure agency and division objectives are met; including implementing feedback loops and participating in QA related activities and reporting.Participates in the development of adult engagement policy and practices for ACP and as requested trains APS case managers, team leaders, team coordinators and state level staff on engagement practices.Performs job delivery and performance of job duties with positive and appropriate customer service delivery to the clients, the public and co-workers as an integral requirement.Performs other duties as requested.
EXAMPLE OF DUTIES
A Bachelor's Degree and at least two (2) years of professional experience in human services or social service programs with adult populations.
