Home Health Aides with Grace and Glory Hospice visit and provide care to patients in their homes. Each patient has a different Plan of Care managed by the RN Case Manager.
Responsibilities and Duties
The Hospice Home Health Aide is responsible for providing personal care under the direction of a registered nurse.

Perform Personal Care for Patients (bathing,dressing,toileting,feeding, etc) according to the Plan of Care

Document care provided according to agency policy through our online system HospiceMD
-
Report any deviations from the Plan of Care to the Case Manager (or designee if not available).

Use durable medical equipment.

Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.

Provide proper care and observation of the patient's skin to prevent breakdown of tissue over bony prominence.

Reports any changes/areas of concern to Case Manager.

Provide respite for patient's family/caregiver when on-site, as appropriate.

Participate in QAPI activities.

Participate in IDG meetings, as appropriate.

NEVER dispense or administer medications.
Job Conditions

The ability to access patients' homes which may not be routinely wheelchair accessible is required.

Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.

Physical activities will include, walking, sitting, stooping, standing and minimal to maximum lifting of patients and the turning of patients.
