Responsibilities
- In need of HHA for live-out weekend Saturday and Sunday. 8hours per shift. start Asap.
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Support medication administration following prescribed protocols while ensuring compliance with HIPAA regulations for patient privacy and confidentiality
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients
- Assist with medical procedures such as catheter care, wound dressing changes, and hospice-related support under supervision
- Document patient progress meticulously in medical records and communicate effectively with healthcare teams to coordinate ongoing care plans
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments preferred
- Demonstrated ability to work with individuals with developmental disabilities or disabilities requiring specialized care
- Knowledge of medical terminology and familiarity with medical documentation processes
- Certification in CPR and first aid required; additional certifications such as hospice care or dementia training are a plus
- Experience operating EMR/EHR systems and managing patient records securely under HIPAA guidelines
- Long-term care experience and home & community care background highly valued
- Strong communication skills, patience, empathy, and the ability to handle sensitive situations professionally
