Job Title: LVN Treatment/Wound Care Nurse

Job Summary:
We are seeking a compassionate, skilled, and detail-oriented Licensed Vocational Nurse (LVN) Treatment/Wound Care Nurse to join our clinical team at our skilled nursing facility in California. The ideal candidate will be responsible for delivering high-quality wound and skin care to our residents in accordance with physician orders, nursing standards, state and federal regulations, and facility policies.
This position plays a vital role in promoting resident healing, preventing skin breakdown, and ensuring compliance with treatment protocols.
Key Responsibilities:
- Provide direct wound and skin care treatments as prescribed by the physician and in accordance with nursing standards.
- Conduct regular and thorough skin assessments and wound evaluations, including measurements, staging, documentation, and photographing (if required).
- Collaborate with interdisciplinary teams including physicians, RNs, physical therapists, dietitians, and other healthcare professionals to develop and implement individualized treatment plans.
- Monitor progress of wound healing and notify appropriate personnel of changes in condition.
- Ensure timely and accurate documentation in the electronic medical record (EMR) system per facility policy and regulatory guidelines.
- Maintain an updated treatment log and skin report.
- Assist with pressure injury prevention programs and conduct staff education as needed.
- Follow infection control protocols and ensure that all treatments are provided in a clean and safe manner.
- Participate in quality assurance (QA) initiatives related to wound care and skin integrity.
- Attend care plan meetings and contribute to resident-centered care planning.
- Ensure compliance with California Department of Public Health (CDPH) regulations, CMS guidelines, and facility policies and procedures.
Qualifications:
- Current and active Licensed Vocational Nurse (LVN) license in the State of California.
- Minimum of 1 year of experience in wound care/treatment nursing in a skilled nursing or long-term care setting preferred.
- Certification in wound care (e.g., WCC, WOCN) is a plus but not required.
- Current CPR/BLS certification.
- Strong assessment and clinical skills, with attention to detail.
- Familiarity with electronic medical records (EMR) systems.
- Ability to work effectively with residents, families, and interdisciplinary teams.
- Knowledge of state and federal regulations governing skilled nursing facilities.
- Excellent organizational, communication, and time management skills.

Pay: $36.00 - $42.00 per hour
