Victory House Transitional Living is a small privately owned business with locations in Lexington. Our goal is to assist clients with a substance abuse or severe mental illness diagnosis to transition into a successful life of sobriety. Clients participate in our long term transitional living program including IOP, TRP, OP, individual therapy, and peer support services.
Responsibilities:
- Provide support and assistance to individuals with substance abuse disorder or sevevere mental illness diagnosis in obtaining needs to foster ongoing independence
- Conduct outreach activities to identify and engage with community resources
- Assess the needs of individuals and develop personalized care plans
- Assist with accessing healthcare services and community resources
- Provide education on health-related topics, such as preventive care and disease management
- Utilize motivational interviewing techniques to promote behavior change and healthy lifestyles
- Collaborate with other healthcare professionals to coordinate care and services
- Advocate for clients within the healthcare system and help navigate social service programs, such as child welfare and Section 8 housing
- Assist with workforce development initiatives, including job training and employment support
- Provide supervision and guidance to individuals in need of behavioral health support
- Mentor individuals in the community, providing guidance and resources for personal growth
Qualifications and Education Requirements ● Bachelor's degree ● 1 year post-grad work experience ● Must have a valid driver’s license
The Preferred Candidate will... ● Be self-motivated ● Have excellent documentation skills ● Possess excellent written and verbal communication skills ● Have a strong working knowledge of computer skills and use of search engines ● Be able to handle crisis situations in a calm and supportive manner ● Maintain a positive, professional attitude toward clients and staff Administrative ● Maintain confidentiality and comply with HIPAA laws ● Adhere to program policies and procedures ● Excellent time management and organizational skills ● Provide holistic and comprehensive case management ● Maintain a professional code of ethics ● Maintain open communication with clients ● Document all sessions with clients ● Maintain a community contact list ● Attend weekly staff meetings ● Participate in training and state regulatory training within 6 months of hire ● Adhere to the state regulation in relation to attending monthly individual and group supervision Client Related ● Create a safe and healthy environment for clients ● Have a sincere commitment to client’s recovery ● Transport clients to appointments when necessary ● Monitor client's emotional well-being and behavior ● Connect client with family and community resources ● Connect client with legal entities when necessary ● Motivate clients toward goal achievement ● Meet with clients weekly ● Help residents with long-term care plan development and educational resources
