Summary:
The Care Partner provides compassionate, Resident-centered care to Residents in the Memory Care neighborhoods (Connections and Haven). This role assists Residents with activities of daily living, promotes independence, and ensures a safe, comfortable, and engaging environment, while always maintaining dignity and respect. Assistance may be total, partial, or stand-by depending upon physical and cognitive ability of Residents.
Duties and Responsibilities:
Assist Residents with activities of daily living and personal hygiene including Bathing, Grooming & Dressing, Toileting, Mobility & Transfers, Nutrition & Hydration, Skin Care, Range of Motion, Vital Signs & Observation, Communication & Documentation, Environmental Safety, etc. Participate/conduct activities and pocket activities as needed.
Ensure highest quality of life for all Residents by safeguarding privacy in compliance with HIPAA regulations, respecting individual preferences, promoting independence, encouraging and assisting with participation in Community activities and events, and maintaining respectful communication at all times.
Assist Residents with safe transfers in and out of bed, chairs, or wheelchairs applying proper techniques and established processes. Based on the Resident's weight and physical abilities, utilizing Team Member assistance or a mechanical lift may be necessary. Provide support with turning and repositioning for Residents who are unable to reposition themselves independently.
Support Residents with personal grooming tasks such as brushing hair, oral care, nail care, and assistance with dressing and undressing as needed. Ensure clothing is properly cared for by folding or hanging clean items and placing soiled items in the hamper.
Assist Residents with bathing as assigned and continuously monitor hygiene, providing support as needed to ensure Residents and their clothing remain clean and well-maintained.
Help maintain the cleanliness, condition, and proper stocking of Resident suites by identifying and reporting housekeeping or maintenance needs and obtaining necessary care supplies from the Community as required.
Maintain accurate Resident records by documenting care provided at the end of each shift and promptly communicating to supervisor any changes or unusual observations related to Residents' physical or psychological well-being.
Monitor residents' skin integrity during care and report any changes to the nurse. Participate in weekly Resident skin checks and wound rounds as needed.
Assist Residents to the dining area or provide in-room meal service as needed. Support meal delivery, offer assistance during dining, and serve snacks between meals. Monitorand documents Residents' food and fluid intake as required.
Remove soiled bedding and replace with clean linens as needed to maintain a comfortable and sanitary environment.
Keep all work and service areas clean, organized, and well-maintained.
Report any suspected or confirmed cases of elder abuse, mistreatment, or neglect in accordance with personnel policies.
Communicate effectively with nursing staff, families, and Team Members. Adhere to all Community policies, safety procedures, and regulatory requirements.
Attend and participate in workshops, seminars, and programs related to the health care industry. Participate in professional development activities as required. Maintain necessary training compliance associated with role.
Other tasks may be assigned as required by the Manager to meet the needs of the Community.
Knowledge, Skills and Qualifications:

High school diploma or equivalent required.

Must possess a current Adult First Aid certification at time of hire and maintain it for the duration of employment.

Certification as a Nurse Assistant preferred. Not required.

Must have excellent verbal communication skills and the ability to interact effectively with Residents, families, Team Members, and communication platforms. Must be able to read and write English and follow verbal and written instructions accurately.

Must be compassionate, patient, and genuinely motivated to provide care and support to seniors.

Ability to collaborate and work effectively in a team-oriented environment.

Work environment may include noise, distractions, and interruptions; must be able to remain calm, composed, and positive in stressful situations.

Team Member is regularly required to stand, walk, and sit, as well as use hands and fingers to handle objects, tools, or controls, and reach with arms at, above, and below shoulder height.

Frequently lifts and moves items weighing up to 35 pounds and must be able to push a minimum of 200 pounds for a distance of at least 200 feet.

May be required to assist with the evaluation of Residents during an emergency.

Willingness and ability to work flexible hours, including evenings, weekends, and holidays.
