Job Summary:
At Parkshore Estates Nursing & Rehabilitation Center, we pride ourselves on providing exceptional care and services to our residents. As our Social Service Director, you will play a vital role in ensuring that our residents receive the highest level of care and support. We are seeking a highly motivated and compassionate professional to join our team and lead our social services department.
Responsibilities:
Provide oversight to the residents, ensuring that all residents receive compassionate and individualized care
Collaborate with other departments, including nursing, therapy, and dietary, to ensure that residents' needs are met
Conduct assessments and develop individualized care plans that focus on resident-centered care and support
Identify and address any social, emotional, or psychological concerns of our residents
Develop and maintain relationships with community organizations and providers to ensure access to resources and services for our residents
Maintain accurate and up-to-date records and reports, including resident assessments, care plans, and service delivery
Requirements:
High School Diploma
Minimum of 2 years of experience in a related role
Strong organizational skills, with the ability to prioritize and manage multiple tasks and responsibilities
Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and staff
Strong analytical and problem-solving skills, with the ability to identify and address complex issues
Ability to work in a fast-paced environment and adapt to changing priorities and situations
