Job Summary:
The Patient Care Coordinator serves as a key liaison between patients and the medical practice, ensuring that inquiries are addressed promptly and accurately. This role involves managing incoming phone calls and text messages to answer general questions, provide updates on referral requests, assist with medication and prescription refill inquiries, schedule or modify appointments, and forward messages to the appropriate parties. The ideal candidate is detail-oriented, organized, and dedicated to delivering excellent customer service while maintaining accurate documentation in the electronic medical record (EMR) system. Bilingual proficiency in English and Spanish is essential for effectively communicating with our diverse patient population.
Responsibilities:
- Patient Communication:
- Field patient inquiries by phone and text message, answering general questions in a courteous and professional manner.
- Provide updates on the status of referral requests and assist patients with understanding next steps.
- Address questions related to medications, prescription refills, and pharmacy-related concerns.
- Scheduling and Appointments:
- Schedule, cancel, or reschedule patient appointments in accordance with practice policies.
- Notify patients of changes to their appointments and provide reminders as needed.
- Message Coordination:
- Document all patient interactions and inquiries accurately in the EMR system.
- Forward messages and patient concerns to the appropriate medical or administrative staff for follow-up.
- Track unresolved issues and ensure they are addressed promptly.
- Administrative Duties:
- Maintain an organized workflow to handle multiple patient inquiries efficiently.
- Update patient contact information and verify records as needed.
- Assist with special projects or administrative tasks as assigned.
Requirements:
- High school diploma or equivalent; additional training in healthcare administration or customer service is a plus.
- Minimum of 1-2 years’ experience in a healthcare or customer service role, preferably in a primary care or family medicine setting.
- Bilingual proficiency in English and Spanish is required.
- Strong organizational and multitasking skills in a fast-paced environment.
- Excellent verbal and written communication skills with a professional and empathetic demeanor.
- Proficiency with electronic medical records (EMR) systems and scheduling software.
- Ability to work both independently and collaboratively with a multidisciplinary team.
- Commitment to maintaining patient confidentiality and adherence to HIPAA regulations.
Physical Demands:
- Prolonged periods of sitting and working on a computer.
- Frequent use of hands for typing, data entry, and phone use.
- Occasional standing, walking, and reaching during the workday.
- Ability to lift up to 15 pounds occasionally (e.g., files or office supplies).
