Overview
An immediate opening for a Bachelor’s Level Mental Health Case Manager to provide paraprofessional services primarily to children under 18 yrs in the Warren AND Monticello area community and schools.
This position requires a self-motivated team player with excellent time management skills and great social skills with all client age groups, including their families and teachers. Previous employment and/or personal experience working with individuals with mental illness and/or mental disability is preferred but not required.
Responsibilities
- Provide outpatient paraprofessional services to clients based on their individualized treatment plans.
- Collaborate regularly with therapists, to ensure services provided are effective and efficient.
- Provide crisis intervention, when necessary.
- Maintain accurate electronic medical records and documentation in compliance with insurance and HIPAA regulations.
Qualifications
- Bachelor’s degree is required, preferably in Social Work, Psychology, or other human services related discipline.
- Excellent time management and communication skills for effective client services and collaboration with healthcare teams, teachers, client families, etc.
- Possess general computer knowledge and navigation skills.

Pay: From $36,000.00 per year
