Home Instead is looking for a compassionate, organized, and relationship-focused Client Care Coordinator to join our team. In this role, you'll help older adults receive exceptional care while building relationships with clients, families, healthcare professionals, and community partners.

What You'll Do
- Meet with prospective clients and families to assess care needs and develop personalized care plans.
- Coordinate services with caregivers and office staff to ensure quality care.
- Conduct client quality assurance visits and follow up on any concerns.
- Maintain regular communication with clients, families, and caregivers.
- Keep accurate client records and documentation.
- Build relationships with hospitals, physicians, senior living communities, and other referral partners.
- Represent Home Instead at community events and networking opportunities.
- Support business development by helping grow referrals and community partnerships.
- Work closely with Scheduling, Operations, and Leadership to provide outstanding client care.
Qualifications
- Associate degree or equivalent work experience preferred.
- At least 2 years of experience in home care, healthcare, client services, care coordination, or a related field.
- Sales, business development, or community outreach experience is a plus.
- Excellent communication, customer service, and relationship-building skills.
- Strong organizational and time management skills.
- Basic computer skills, including Microsoft Office.
- Valid driver's license, reliable transportation, current auto insurance and willingness to travel locally.
Service Area
This position requires regular local travel throughout our Los Angeles service area, including East Los Angeles, Hollywood, Chinatown, Echo Park, Silver Lake, Westlake, Mid-Wilshire, Koreatown, Downtown Los Angeles, Whitley Heights, Angelino Heights, Larchmont Village, and surrounding communities.

Pay: $60,000.00 - $65,000.00 per year
