Service Areas: Plymouth, New Hope, Crystal, St. Louis Park, Wayzata, Hopkins, Mound, Minnetonka, Maple Plain, Buffalo.
What you bring; Physical Therapist Must Haves:
- Bachelor's Degree or more in Physical Therapy from an accredited professional PT education program
- Active License on the Minnesota Board of Physical Therapy
- Current CPR Certification
- Ability to travel up to 20 miles as needed
Physical Therapist Duties:
- Provide physical therapy services to clients according to a physician’s written orders. Perform direct physical therapy treatment.
- Assess and evaluate therapeutic/rehabilitative/functional status.
- Develop a plan of care and treatment making sure plan of care is consistent with assessment.
- Initiate physical therapy program and instructs other personnel and/or family members and caregivers in specific phases of physical therapy with which they may assist with the client care.
- Participate in discharge planning, as appropriate.
- Report client’s response to treatment or changes in condition to the Director of Nursing and physician.
