What You'll Be DoingRestore Mobility And Independence
- Perform physical therapy evaluations and treatment visits in patient homes
- Develop treatment plans focused on strength, mobility, endurance, balance, and fall prevention
- Help patients recover following hospitalization, surgery, illness, or injury
- Recommend assistive devices and home safety modifications
- Educate patients and caregivers to maximize long-term success
Collaborate Across The Continuum
- Partner with nurses, Occupational Therapists, Speech Language Pathologists, physicians, and case managers
- Support continuity of care and exceptional outcomes
- Complete documentation that supports quality and regulatory compliance
- Serve as a trusted resource for patients and families
Required Qualifications
- Active Minnesota Physical Therapist license
- Strong clinical assessment and patient education skills
- Ability to work independently and manage a visit-based schedule
- Reliable transportation, valid driver's license, and current auto insurance
Preferred Qualifications
- Home health experience
- Experience with geriatric or medically complex populations
- Passion for relationship-based, patient-centered care
