Medicare Certified Home Health Care Agency, Iron Point Home Health Care, Inc. , is looking for Home Health Occupational Therapist in Northern California. We are serving the healthcare needs of our community by providing services needed in order to allow an individual to remain safely and comfortably in their home. The OT, under physician orders, is responsible for the treatment/management of clients needing OT services in the home setting. The therapist will provide in-home, one-on-one care to a predominately geriatric population. This home health position will give providers the opportunity to apply their therapy skills, while allowing for a flexible schedule. Responsibilities include proficiency in OT management/treatments. The ability to work independently required. Excellent communication and interpersonal skills essential.
Job Description:
Records clinical/progress notes in the patient's clinical record weekly. Instructs the family in the client/patient's total speech therapy program and evaluates the home environment. Instructs other involved agency personnel in the care of client/patients receiving physical/occupational therapy. Plan for patient's discharge, prepares discharge instructions. Computer Skills for documentation and able to drive to Client’s home.
-Provide therapy services consistent with the patient’s Plan of Treatment.
-Observe patient during treatment, compile data and report patient’s progress.
-Act as a liaison and maintain contact with patients, family and Clinical Team to ensure effective care coordination.
-Provide hands-on care and education of the patient.
-Self-directed and able to work with minimal supervision.
-Demonstrates excellent observation, problem solving, verbal and written communication skills.
-Shows ability to organize and prioritize workload independently.
-Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
