Duties
- Dispense prescriptions accurately while adhering to all regulatory guidelines and hospital policies.
- Provide comprehensive patient consultations, including medication counseling, assessments, and education on proper medication use.
- Manage inventory levels efficiently, including ordering, stock rotation, and compounding medications when necessary.
- Perform patient assessments to determine appropriate therapy plans, especially in specialized areas such as LTC (Long-Term Care), critical care, pediatrics, and acute care settings.
- Administer immunizations and perform IV infusions using aseptic techniques while maintaining strict compliance with HIPAA and safety standards.
- Utilize pharmacy software, EMR (Electronic Medical Records), and EHR (Electronic Health Records) systems for documentation, medication tracking, and medical management.
- Collaborate with healthcare teams on pain management strategies, medical management plans, and medication administration protocols.
- Conduct patient care activities including medication reconciliation, patient assessment, and education to enhance overall health outcomes.
- Supervise pharmacy technicians and support staff to ensure efficient workflow and compliance with all pharmacy practices.
- Maintain accurate records of compounding medications, inventory management logs, and incident reports related to pharmacy operations.
Skills
- Extensive knowledge of LTC pharmacy practices, critical care protocols, and hospital pharmacy procedures.
- Proven clinical pharmacy experience with a focus on patient service and assessment in diverse healthcare settings.
- Strong understanding of medical terminology, physiology, anatomy, and pharmacology principles relevant to medication therapy management.
- Skilled in the use of pharmacy software systems along with EMR/EHR platforms for seamless documentation and communication.
- Experience in compounding medications and performing aseptic techniques for IV insertion and infusion therapies.
- Ability to manage inventory effectively while ensuring medication safety standards are met at all times.
- Knowledge of immunizations administration procedures and pain management strategies within acute care environments.
- Prior pharmacy residency or specialized training in hospital or critical care pharmacy is highly desirable.
- Excellent communication skills with the ability to educate patients about their medications clearly and confidently.
