About the Opportunity
AACI is currently seeking a compassionate and dedicated Enhanced Care Management (ECM) Case Manager to support high-risk Medi-Cal members with complex medical, behavioral health, and social needs. In this role, you will provide outreach, care coordination, advocacy, and case management services to help clients access healthcare, housing, behavioral health resources, and community-based programs that promote long-term stability and wellness.
Qualifications
Required:
- Bachelor’s degree in Social Work, Psychology, Public Health, Human Services, or a related field (or equivalent combination of education and experience)
- Minimum of one year of experience in case management, care coordination, social services, or a related field
- Knowledge of community resources, public benefits, and social service systems
- Strong communication, organizational, and interpersonal skills
- Valid California Driver’s License and proof of auto insurance
Preferred:
- Bilingual in Spanish, Tagalog, Vietnamese, Mandarin, or Cantonese
- Experience working with high-risk, underserved, or vulnerable populations
- Familiarity with Medi-Cal, CalAIM, or Enhanced Care Management programs

Pay: $25.00 - $28.50 per hour
