We are hiring Targeted Case Managers (TCMs) to work with both youth and adult clients who are receiving behavioral health services. TCMs help identify each client’s unique needs and connect them with appropriate services within our agency and the community.

Position Summary
We are seeking a dedicated Targeted Case Manager to join our team and provide high-quality case management services to adults, adults in recovery, children, adolescents, and families. This role involves coordinating care, advocating for client needs, and linking clients to essential resources to help them achieve their goals for stability and recovery.
Key Responsibilities
- Provide community-based targeted case management to eligible clients
- Complete comprehensive assessments, individualized service plans (ISPs), and documentation in accordance with Medicaid and agency guidelines
- Coordinate services across multiple systems (behavioral health, education, legal, housing, employment, etc.)
- Support clients in achieving mental health, substance use recovery, and personal wellness goals
- Monitor client progress and update service plans in collaboration with clients and multidisciplinary teams
- Maintain accurate and timely documentation in electronic health records (EHR)
- Travel within the community to meet with clients and provide services in home, school, or community settings
Requirements
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related human services field
- Must have (or be eligible to obtain) TCM certification through the Kentucky Department for Medicaid Services (KY DMS)
- Minimum of one year of experience working with target populations (adults, children, or families with behavioral health needs)
- Valid driver’s license, reliable transportation, and current auto insurance
- Strong interpersonal, communication, and organizational skills
- Ability to work independently and as part of a team
Preferred Qualifications
- Experience working with both adult and youth populations
- Knowledge of Medicaid billing and community resources in Kentucky
- Bilingual (English/Spanish) a plus

Pay: $23.00 - $27.00 per hour
