Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to promote independence and dignity
- Monitor and observe patient conditions, including vital signs, mental status, and overall health status, documenting changes accurately in electronic health record (EHR) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavior management strategies
- Aid with medical procedures such as catheter care, wound dressing assistance, and end-of-life care with sensitivity and professionalism
- Assist with light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
- Use patient transfer methods safely with equipment such as Hoyer lifts or transfer belts to prevent injury for both patient and caregiver
Skills
- Strong knowledge of medical terminology and documentation practices for accurate record keeping
- Experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring specialized care
- Proficiency in using EMR (Electronic Medical Record) systems and EHR systems for efficient documentation
- Excellent communication skills for effective client interaction and collaboration with healthcare teams
- Ability to perform heavy lifting safely when necessary to assist patients or move equipment
- Certification in First Aid, Basic Life Support (BLS), CPR, and knowledge of home health care assistance protocols
- Familiarity with home & community care standards, hospice care principles, and long-term care practices
