We are seeking a compassionate and experienced Occupational Therapist (OT) to join our growing team and provide high-quality therapy services to clients throughout the New Haven and Valley areas of Connecticut.

Evaluate patients' functional abilities and develop individualized treatment plans.
Provide occupational therapy services based on each patient's clinical needs and goals.
Help patients improve independence with activities of daily living, mobility, safety, and functional skills.
Monitor patient progress and adjust treatment plans as appropriate.
Educate patients, families, and caregivers on therapeutic techniques and home programs.
Collaborate with physicians, nurses, care managers, and other healthcare professionals.
Complete accurate and timely clinical documentation.
Maintain compliance with applicable federal, state, and organizational requirements.
Travel throughout the assigned New Haven and Valley service area as needed.
Current and valid Connecticut Occupational Therapist license.
Graduate of an accredited Occupational Therapy program.
Current CPR/BLS certification, as required.
Strong clinical assessment and documentation skills.
Excellent communication and interpersonal skills.
Ability to work independently and manage a flexible schedule.
Previous home health, community-based, outpatient, or geriatric experience is a plus.
Reliable transportation for community-based assignments.
