As a Licensed Practical Nurse, you will play a vital role in delivering high-quality patient care and contributing to a positive and supportive environment for our residents. Our state-of-the-art facility offers a wide range of services and amenities, including short-term rehabilitation, long-term care, and memory care.
Responsibilities:
- Provide direct patient care, including administering medications, treatments, and procedures under the supervision of a licensed RN or MD
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor patient progress and report any changes or concerns to the nursing team
- Participate in shift reports and communicate effectively with colleagues to ensure seamless care transitions
- Maintain accurate and detailed records of patient care and treatment
- Perform routine tasks, such as vital sign checks, wound care, and range of motion exercises
Requirements:
- Current LPN licensure in the State of Michigan
- High school diploma or equivalent required; associate's degree or higher preferred
- Minimum of 0 years of experience in a long-term care or rehabilitation setting
- Ability to work effectively in a fast-paced environment and prioritize tasks
- Excellent communication and interpersonal skills
