We're part of an established, regionally respected post-acute care organization serving communities across the East Coast. Our mission centers on redefining how healthcare is delivered, investing in the people who deliver it, and building a workplace culture where team members feel as supported as the clients they serve.We're currently looking for an Admissions & Outreach Coordinator to help grow our presence in the community and guide new clients through a smooth, well-supported transition into our care.
About the Role
This position sits at the intersection of relationship-building and operations, driving referrals, managing the admissions pipeline, and acting as the go-to contact for prospective clients, their families, and the community partners who refer them. Success in this role means blending a sales mindset with genuine, client-first customer service, all while keeping every step compliant and well-documented.
What You'll Do
Coordinate Admissions Day-to-Day
- Represent bed availability and upcoming admissions during daily leadership huddles.
- Keep admission paperwork moving and complete.
- Track how tours are converting and report on outcomes.
- Walk the building daily to assess readiness and flag opportunities for improvement.
- Own the standard for getting rooms move-in ready.
- Stay involved in the discharge follow-up process.
Build the Referral Pipeline
- Cultivate ongoing relationships with organizations throughout the community.
- Make regular visits to assisted living communities, senior centers, high-rise senior housing, and physician offices.
- Show up at local networking events and community programs as a face of the organization.
- Educate referral sources on what we offer, including specialty programs and outcomes.
- Keep a consistent, visible presence across your assigned territory.
Drive Growth Strategically
- Partner with the hospital liaison and centralized admissions team to hit census and admission targets.
- Watch daily census trends and get ahead of dips before they happen.
- Use referral and discharge data to sharpen outreach strategy over time.
- Contribute to marketing planning and rollout.
Deliver on the Experience
- Be the bridge between families and facility leadership throughout the admissions journey.
- Lead tours and give prospective clients a personal, consultative experience.
- Make sure every first impression counts.
- Handle concerns quickly, professionally, and with care.
- Hold every interaction to HIPAA confidentiality standards.
What You Bring
- 2+ years in healthcare admissions, marketing, business development, or case management.
- Working knowledge of Medicare, Medicaid, and managed care reimbursement structures.
- Strong communication, negotiation, and relationship-building instincts.
- A track record of hitting performance targets.
- A valid driver's license and reliable transportation — this role gets you out into the community.

Pay: From $55,000.00 per year
