As a Physical Therapist Assistant, you will provide skilled in-home therapy services under the direction and supervision of a licensed Physical Therapist. You will help patients improve strength, mobility, safety, and independence while supporting their ability to remain safely in their homes. You will collaborate closely with nurses, therapists, and other members of the interdisciplinary team to deliver exceptional, patient-centered care.

Provide skilled physical therapy treatments in patients' homes under the direction of a licensed Physical Therapist
Implement individualized treatment plans designed to improve mobility, strength, balance, endurance, and safety
Assist patients with therapeutic exercises, gait training, transfer training, and functional mobility activities
Instruct patients and caregivers on home exercise programs and safety techniques
Observe, document, and report patient responses to treatment and progress toward established goals
Communicate changes in patient condition to the supervising Physical Therapist and interdisciplinary care team
Assist with recommendations regarding adaptive equipment and home safety modifications
Maintain accurate and timely documentation in accordance with agency policies and regulatory requirements
Collaborate with nurses, occupational therapists, speech therapists, social workers, and case managers to coordinate care
Promote patient independence and quality of life through compassionate, patient-centered care

Graduate of an accredited Physical Therapist Assistant program
Current Physical Therapist Assistant license in the State of Iowa
Current CPR certification
Valid driver's license, reliable transportation, and current auto insurance
Home health experience preferred, but not required
Strong communication, organizational, and clinical skills
Ability to work independently while maintaining collaboration with the healthcare team
