Location: Rockville, Maryland
Facility: Shady Grove Nursing and Rehabilitation Center
Shift: Monday – Friday | 8:00 AM – 4:00 PM
Schedule: 5×8-Hour Shifts

Key Responsibilities
- Perform comprehensive patient screenings, evaluations, and assessments.
- Develop, implement, and modify individualized treatment plans based on patient needs and goals.
- Deliver skilled physical therapy services to restore function, mobility, strength, and independence.
- Prepare timely and accurate documentation of evaluations, treatments, and patient progress.
- Develop discharge plans and provide patient and caregiver education.
- Supervise Physical Therapist Assistants (PTAs) and Rehab Technicians in accordance with state practice acts.
- Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
- Ensure compliance with facility policies, state and federal regulations, infection control, and patient safety standards.
Qualifications
- Graduate of an APTA-accredited Physical Therapy program (DPT, MSPT, or BSPT).
- Active Maryland Physical Therapist license (or eligibility to obtain prior to start).
- Previous experience in Skilled Nursing Facility (SNF), Rehabilitation, Long-Term Care, or Acute Care preferred.
- Strong clinical assessment, documentation, and communication skills.
- Ability to work independently while contributing effectively within a multidisciplinary team.
- Passion for delivering high-quality, patient-centered rehabilitation services.
