Avir at Rose Trail and Avir at Petal Hill are seeking a Nurse Practitioner to provide on-site clinical care between both facilities in Tyler, Texas.
This is a facility-based role focused on managing post-acute and long-term care residents within a skilled nursing environment. The Nurse Practitioner will work closely with physicians and nursing leadership to support strong clinical outcomes and reduce hospitalizations.
Key Responsibilities
- Provide comprehensive medical care to residents
- Perform assessments, follow-ups, and manage acute changes in condition
- Collaborate with physicians and interdisciplinary teams
- Participate in care planning and clinical meetings
- Ensure timely and accurate documentation
- Support quality initiatives and rehospitalization reduction efforts
Qualifications
- Active Texas Nurse Practitioner license
- Nursing home or long-term care experience required
- Strong clinical assessment and decision-making skills
- Ability to work independently within a structured clinical model
