North Eastern Services is seeking outgoing, responsible, and self-motivated individuals to join our team as a Day Support Professional. In this role, you provide one-on-one, community-based mentorship to individuals with developmental and physical disabilities.
This is more than just supervision; you are an active advocate and guide. From 9:00 AM to 3:00 PM, you will spend your day out in the community, helping the individual you support explore new hobbies, practice social skills, and engage with their environment in meaningful ways. By stepping into this role, you aren't just filling a shift—you are helping someone build confidence, achieve their personal independence goals, and experience the full richness of community life.
Key Responsibilities
- One-on-One Community Engagement: Spend your shift facilitating personalized outings that align with the individual's interests and goals. You act as a mentor, helping them navigate social interactions and build real-world skills outside the home.
- Safe Community Transportation: Provide safe, reliable transportation to and from community activities, appointments, and events using your personal vehicle. Ensuring the safety and comfort of the individual during travel is a primary duty.
- Goal Implementation & Skill Building: Actively assist the individual in working toward specific goals outlined in their service plan, such as improving communication, building life skills, or increasing independence in public spaces.
- Documentation & Reporting: Maintain accurate, timely, and detailed records of daily activities, progress toward goals, and any required medical or incident documentation (such as MAR/GERs) to ensure compliance with state and DSPD requirements.
- Health, Safety, & Advocacy: Ensure the individual’s safety at all times. This includes following all behavior support plans, advocating for the individual’s rights, and maintaining a professional demeanor while interacting with community members and families.
- Team Communication: Coordinate effectively with supervisors and the broader NES team to ensure consistent, high-quality care that supports the mission of North Eastern Services.
Requirements
- Must be at least 21 years of age
- Must pass a drug screening
- Must pass a criminal background check
- Must be able and willing to lift over 50 lbs
Pay: $14.00 - $17.00 per hour
