Physical Therapist
- Responsible for the evaluation, treatment, reassessment, and discharge of all referrals they accept.
- Promptly respond to referrals
- Write the plan of care for PTA to follow.
- Conference with PTA for patient care.
- Communicate with the home health agency to report status at evaluation, reassessment, discharge, and if any irregularities arise.
- Schedule their visits.
- Complete their invoice.
- Complete all documentation within 36 hours of visit
Pay: $60.00 - $100.00 per hour
