Paragon Home Care is seeking an experienced Field Coordinator who will be a strong addition to our caring, dedicated, and mission‑driven team. This role is essential in supporting high‑quality, compliant non‑skilled home care services. This position requires a reliable professional who excels in customer service, communicates effectively in both English and Spanish, and thrives in a collaborative team environment committed to caring for others.
Key Responsibilities
- Conduct initial assessments non‑skilled home care consumers.
- Complete 90‑day supervisory visits in accordance with regulatory and agency requirements.
- Perform pop‑up field visits to ensure service quality, caregiver compliance, and consumer satisfaction.
- Install FOB devices in consumer homes and verify proper functionality.
- Provide EVV training to caregivers, ensuring they understand clock‑in/clock‑out procedures and troubleshooting basics.
- Document all field activities promptly and accurately.
- Run required reports to track visit compliance.
- Schedule assessments, supervisory visits, and FOB device installation, and EVV trainings.
- Perform un-scheduled pop-up visits.
- Download, upload, and organize documents within the agency system.
- Update consumer and caregiver records to maintain accurate and compliant data.
- Communicate effectively with all agency departments.
- Support team members with field coordination needs and contribute to a collaborative work environment.
Required Qualifications
- High School Diploma (required).
- Bilingual in English and Spanish (required).
- Valid driver’s license with a good driving record (required).
- Excellent communication and customer service skills.
- Ability to work independently in the field and collaboratively with the office team.
- Strong organizational skills and comfort using technology, mobile apps, and agency systems.
- Ability to manage multiple tasks, deadlines, and changing priorities.
Work Environment
This role requires frequent travel to consumer homes, occasional office time, and consistent communication with agency departments. The Field Coordinator must be comfortable working in varied home environments and interacting with diverse populations.
