Position Overview
As a Registered Nurse, you will act as a key clinical resource across the full spectrum of care. In this versatile field role, you will perform initial admissions assessments, routine/urgent PRN visits, and crucial Service Intensity Add-on (SIA) care during end-of-life transitions across private homes, hospitals, and skilled nursing facilities.
Key Responsibilities
- Comprehensive Care Delivery: Act as a primary field resource for hospice care by conducting initial admissions, routine care visits, urgent PRN responses, and Service Intensity Add-on (SIA) visits near end-of-life.
- Admissions & Assessment: Evaluate hospice eligibility per Medicare guidelines, establish individual plans of care, and guide patients and caregivers as they begin hospice services.
- Patient & Caregiver Education: Educate patients, caregivers, and community partners on hospice philosophy, symptom management, and care expectations.
- Interdisciplinary Collaboration: Partner closely with Clinical Leadership, physicians, Intake Coordinators, and Liaisons to ensure seamless care coordination and rapid symptom response.
- Documentation Excellence: Complete accurate, timely, and compliant EMR documentation (HospiceMD) to support quality care, audit readiness, and regulatory compliance.
Qualifications & Requirements
- Licensure: Active California Registered Nurse (RN) license in good standing.
- Education:
- Associate Degree in Nursing (ADN) required
- Bachelor of Science in Nursing (BSN) preferred
- Experience: Minimum of 1 year of clinical nursing experience (Hospice or Home Health experience preferred).
- Skills: Strong clinical assessment, symptom management, clear communication, and adaptability to changing visit priorities (Admissions, PRN, Routine, SIA).
- Tech-Savvy: Proficiency with electronic medical records (experience with HospiceMD is a plus).
- Mobility: Valid California driver’s license, reliable transportation, and willingness to travel.
- Shift Hours: 11:30 AM – 8:00 PM or 12:00 PM – 8:30 PM
- Weekend availability required
