SHIFTS NEEDED:
Day shift; 6:00 AM - 6:30 PM ; Friday , Saturday , Sunday
REQUIREMENTS:
- 18 years or older
- High school diploma or equivalent
- Graduate of accredited LPN or RN School of Nursing
- Current valid LPN/LVN / RN license in the state employed
- Valid CPR certification
ESSENTIAL FUNCTIONS:
- Provides direct care and or assists with care as directed and consistent with their scope of practice and competency. Makes rounds with physicians and other team members as needed. Participates in resident evaluation and care planning activities, reviews and revises residents’ assessments and care plans as needed to the extent permitted by . Coordinates residents’ care activities and communicates changes to other team members and resident family / significant other.
- Reviews, transcribes, communicates and implements physician orders, obtaining as required by state regulations or company policy. Communicates with resident’s physician. Assists with emergencies, administers cardiopulmonary resuscitation.
- Documents care performed and observations of resident status in the clinical record promptly by utilizing standard clinical documentation guidelines as required by the company, and local, state and federal rules and regulations. Monitors completeness and accuracy of own clinical medical record entries and those of assigned LPN and Nurse Aides daily.
- Participates in medication management to promote optimal safety and effectiveness for residents by:
- Administering medications as ordered, including next scheduled admission/readmission.
- Notifying Supervisor if medications are not available;
- Preparing, administering and documenting medications as prescribed;
- Observing and reporting resident responses to medication;
- Identifying and promptly communicating adverse drug reactions;
- Maintaining narcotic records within scope of practice;
- Completing medication interchange and insurance authorizations as assigned;
- Ordering or arranging for ordering of pharmaceuticals;
- Cleaning, organizing and replenishing supplies on medication/treatment carts after each use and prior to shift change.
- Initiates and assists consultants while consultants are providing services. Reviews documentation from consultant visit and follows up on recommendation(s).
- Assists with follow up on of qualitative and quantitative medical record audits.
- Communicates and implements pharmacy review/drug review recommendation as assigned.
- Assists with clinical discharge process.
- Performs other clinical care services within scope of practice for additional certifications.
- Ensures punctuality and regular attendance for assigned shifts.
OTHER FUNCTIONS:
- Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, applicable federal and state laws, and applicable professional standards.
- Attend Staff meetings and conducts necessary training in accordance with state regulations.
- Should be patient, polite and be able to communicate effectively with residents, their family members, physicians and other staff.
- Performs other tasks as assigned.
