Pharmacist – Hospital Outpatient | Job Description

Schedule: Monday–Friday | 10:00 AM–6:00 PM | 35 hours/week
Job Summary
Facility is seeking an experienced Hospital Outpatient Pharmacist to review, dispense, and monitor prescriptions while providing safe, effective, and patient-centered pharmaceutical care. The Pharmacist will work closely with providers, patients, caregivers, and pharmacy technicians in a high-volume outpatient pharmacy environment.
Key Responsibilities
- Review patient prescriptions and medication profiles for accuracy, appropriateness, allergies, contraindications, and drug-drug interactions.
- Dispense prescriptions in compliance with NYS laws, hospital policies, and pharmacy procedures.
- Monitor drug therapy and communicate with providers to clarify prescriptions and resolve medication-related concerns.
- Provide clinical consultation and recommend cost-effective or therapeutically appropriate alternatives.
- Counsel patients and caregivers regarding medications, proper use, potential side effects, and precautions.
- Supervise and provide guidance to pharmacy technicians.
- Process prescriptions and maintain familiarity with billing procedures and third-party contracts.
- Manage prescription inventory and support efficient pharmacy operations.
- Work with high-volume prescription workflows in a retail/outpatient pharmacy setting.
- Utilize pharmacy dispensing systems and EMR systems, preferably EPIC Willow Ambulatory.
Required Qualifications
- Bachelor’s Degree or higher in Pharmacy.
- Minimum 1 year of Pharmacist experience.
- Valid New York State Pharmacist License issued by NYSED with Primary Source Verification.
- Strong experience in prescription review, drug therapy monitoring, clinical consultation, and medication counseling.
- Experience with billing procedures and pharmacy operations.
- Strong English communication skills.
Preferred Experience
- High-volume outpatient/retail prescription experience.
- Pharmacy dispensing system experience.
- EPIC Willow Ambulatory orders experience.
- EMR experience.
- Inventory management.
- Pharmacy technician supervision.
- Familiarity with sterile processing and IV room workflows.
Important: This position does not require IV compounding or IV Room workflow experience.

Pay: $90.00 - $100.00 per hour
