Job Summary
The Clinical Pharmacy Care Coordinator will function in conjunction with the clinical pharmacy team and the Quality Team to deliver care within a Federally Qualified Health Center. Duties include performing as a reliable, self-motivated member of the team and assisting in the administration and maintenance of an efficiently operated clinic. This role is essential in promoting preventive care, addressing health disparities, and improving outcomes for patients. The Clinical Pharmacy Care Coordinator must demonstrate a sincere dedication and loyalty to the mission, vision and core values of BRMC. Clinical Pharmacy visits currently consist of Medicare Annual Wellness Visits, but may be expanded to include acute visits or focused clinics. This position may involve assignments at satellite locations as needed.
Essential Job Duties
- Conduct outreach to schedule clinical pharmacy visits and other visits as necessary, prioritizing high-risk patients by using appropriate portals and insurance information.
- Conduct pre-visit planning for efficient visits.
- Complete standardized rooming and perform clinical intake, data collection, and documentation of patient care provided. This may include administering and documenting standardized screening tools (depression, fall risk, cognitive assessment, social determinants of health).
- Administer immunizations under the direction of a provider.
- Assist in examination and treatment of patients. Perform necessary test(s) as directed by provider. [KB1]
- Collect, label, and prepare specimens for lab. [KB2]
- Support accurate, timely, and thorough clinical documentation, appropriate coding practices, and compliance with quality reporting requirements.
- Assist with routine maintenance activities including, but not limited to, the following: cleaning equipment, stocking exam rooms, organizing clinical work area, etc.
- Complete work activities successfully through effective management of time, tasks and available resources. Remain vigilant in the performance of duties.
- Represent the organization in a professional manner through appropriate appearance, respectful communication, and compassionate interactions. Demonstrates the ability to articulate and promote the value of the healthcare services provided, including the organization’s commitment to quality care, patient-centered service, and improved health outcomes for the community.
- Perform other necessary duties as assigned by the Clinical Pharmacist to meet the goal of providing quality health care services.
Compensation Range: $20.76-$24.11/hr
