Job Summary:
As a Home Health Registered Nurse, you will provide direct nursing care to patients in their homes, ensuring individualized care plans are followed and updated as needed. You’ll collaborate with the interdisciplinary team to promote the best possible health outcomes and help patients maintain their independence and well-being.
Responsibilities:
- Provide skilled nursing care and follow-up visits based on established care plans.
- Monitor patient conditions and report changes to physicians and care team.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on disease management, medication, and home safety.
- Document visits and clinical findings accurately and timely using OASIS (experience preferred).
- Collaborate with physicians, therapists, and other members of the care team to coordinate services.
Qualifications:
- Must be licensed as a Registered Nurse in the state of Wisconsin.
- Valid Wisconsin driver’s license and reliable transportation with current auto insurance.
- Prior Home Health Nursing experience required.
- Familiarity with Medicare guidelines and OASIS documentation is preferred.
- Ability to work independently and manage time effectively.
- Strong communication, organizational, and critical thinking skills.

Pay:
Per visit rate – $47.00/visit
