SUMMARY
The Medicare Scheduling Coordinator is responsible for coordinating patient visits, maintaining scheduling records, and ensuring timely communication of schedule changes with both office and field staff. This role also requires accurate and consistent data entry and maintenance in Soneto and Brightree to support efficient operations and quality patient care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Coordinate and schedule field staff assignments, ensuring all patients are assigned to qualified staff in accordance with RN/PT/OT/ST assessments, physician orders, and HHA requirements.
- Communicate daily with field staff and document confirmations and/or schedule changes in the scheduling software.
- Schedule new cases to ensure compliance with agency regulations, follow up on scheduling issues, and create schedules in the EMR once approved by the supervisor.
- Notify the Clinical Manager whenever qualified staff are not available as needed.
- Confirm visits and timesheet information for payroll processing and manage paycheck distribution weekly.
- Serve as a liaison between referral sources and referral portals, coordinating the intake process and maintaining communication with marketers, patients/caregivers, ordering physicians, and other home health agencies. This includes contacting physicians to confirm acceptance of the care plan.
- Update standard reports regularly (e.g., weekly reports and daily Medicare updates).
- Provide on-call services when necessary.
- Maintain the current client roster and patient reports for management review.
- Create patient and client charts to ensure accurate record retention, including scanning documents into the EMR.
- Performs all other duties as assigned.
