Position Summary:
We are seeking a Registered Nurse (RN) to support the delivery of high-quality non-medical home care services through caregiver oversight, client assessments, documentation, and compliance activities. This position is instrumental in supporting Georgia Medicaid Waiver programs (e.g., CCSP, SOURCE, EDWP). The role requires strong clinical judgment, the ability to independently support caregivers in the field, and active participation in documentation and care planning processes.
Key Responsibilities:
- Conduct initial and ongoing in-home assessments of clients receiving personal support or companion services.
- Perform supervisory visits and wellness checks to ensure caregiver compliance, service quality, and client safety.
- Between in home assessments and supervisory visits someone could be expected to complete up to 25 visits a week.
- Observe and report any changes in client condition to the supervising RN or care team.
- Collaborate with the RN and case management team to create, amend, and co-sign Individualized Service Plans (ISPs) based on client needs and documentation received.
- Review and maintain clinically accurate documentation in the agency’s CRM in accordance with Medicaid and state guidelines.
- Provide field-based coaching and training to non-clinical caregivers (e.g., PCAs/CNAs) on ADLs, infection control, safety techniques, and client-specific needs.
- Participate in agency audits, quality assurance activities, and client satisfaction initiatives.
- Serve as a communication liaison between caregivers, clients/families, and clinical leadership.
- Co-sign documentation related to supervisory visits and care oversight, as appropriate.
- Assist with onboarding and cross-training of new LPNs or field staff as needed.
- Attend internal meetings to report caseload updates and assist in team-based decision-making.
- Maintain strict adherence to HIPAA and confidentiality standards.
Qualifications:
- Active Registered Nursing Degree in the state of Georgia
- CPR Certification (BLS for Healthcare Providers)
- At least 3–5 years of experience in home care, HCBS, or waiver-based services preferred
- Strong working knowledge of Georgia Medicaid Waiver programs (CCSP, SOURCE, EDWP, ICWP) preferred
- Excellent documentation, communication, and organizational skills
- Ability to work independently in the field and manage multiple priorities
- Reliable transportation, valid driver’s license, and proof of auto insurance
- Comfortable training and coaching non-clinical caregivers
- Willingness to travel within and occasionally outside Metro Atlanta, as needed

Base Pay: $85,000.00 - $90,000.00 per year
