About the Role
A&B Home Services, Inc is seeking compassionate RN/LPN nurses to support our GAFC and AFC programs. This is a non-skilled, community-based role focused on supervisory visits, assessments, and care coordination — no skilled nursing procedures required.
This position is ideal for nurses looking for flexibility, independence, and additional income without the demands of traditional home health.
Key Responsibilities
- Conduct in-home supervisory visits for GAFC/AFC participants
- Complete new patient admissions and initial assessments
- Develop and update care plans in accordance with MassHealth guidelines
- Provide clinical oversight and guidance to caregivers
- Monitor participant condition and report changes
- Maintain accurate and timely documentation
- Communicate with case managers and interdisciplinary team
- Ensure compliance with GAFC/AFC program requirements
Qualifications
- Active Massachusetts RN or LPN license (required)
- Experience in home care, GAFC/AFC, or community-based care preferred
- Knowledge of MassHealth regulations is a plus
- Strong organizational and documentation skills
- Ability to work independently and manage schedule
- Reliable transportation required
Compensation
- Per visit pay (competitive rates based on experience)
- $100 per new patient admission

Work Location:

Admissions paid at $100 – higher than standard GAFC/AFC rates
LPN oversight visit $30-35 per visit, RN regular visit $ 45-50
Pay: $35.00 - $50.00 per hour
