Oak Medical is seeking a Nurse Practitioner or Physician Assistant to provide onsite care for adult and geriatric patients across our post-acute and long-term care partner communities. You'll work alongside experienced physicians and interdisciplinary care teams while enjoying a predictable weekday schedule and strong clinical support.

Your Impact
As a valued member of our clinical team, you will:

Provide primary care services for adult and geriatric patients

Diagnose and manage acute and chronic medical conditions

Develop and implement individualized treatment plans

Prescribe medications and perform appropriate clinical procedures

Coordinate care with facility teams and specialty providers

Communicate effectively with patients, families, and caregivers

Maintain accurate and timely documentation

Help improve quality of life and health outcomes for the patients you serve
Qualifications
Required:
-
Licensed Nurse Practitioner or Physician Assistant
-
Active DEA registration
-
Current BLS certification
-
Strong clinical assessment and diagnostic skills
Preferred:
-
Experience in geriatrics, skilled nursing, long-term care, hospital medicine, or post-acute care

Leadership experience

Strong organizational and time-management skills
