Are you an organized, detail-oriented RN with a passion for clinical accuracy and resident advocacy? Brent B. Tinnin Manor, a 60-bed skilled nursing community in the heart of Reynolds County, is seeking a dedicated MDS Coordinator (Registered Nurse) to lead our resident assessment and care planning processes.
As our MDS Nurse, you will be the bridge between clinical care, regulatory compliance, and reimbursement. You will ensure that every resident receives a highly personalized care plan while accurately capturing the complex nursing and therapy services our team provides daily.
What You Will Do
- Coordinate the MDS Process: Oversee the scheduling, completion, and timely electronic submission of the Minimum Data Set (MDS) in accordance with state and federal regulations.
- Drive Interdisciplinary Care Planning: Lead care plan meetings with families, therapists, and interdisciplinary team to ensure holistic, individualized resident care.
- Ensure Reimbursement Accuracy: Review clinical documentation to ensure it accurately reflects the resident’s acuity level, capturing appropriate case-mix and PDPM (Patient Driven Payment Model) categories.
- Quality Assurance & Auditing: Participate actively in Quality Assurance and Performance Improvement (QAPI) initiatives to maintain high clinical standards and regulatory compliance.
- Assess Changes in Condition: Monitor residents for significant changes in health or functional status to trigger appropriate modifications to their MDS assessments and care goals.
What We Are Looking For
- License: Current, unrestricted Registered Nurse (RN) license in the state of Missouri (or compact license).
- Experience: Prior experience as an MDS Coordinator or working extensively with RAC-CT certification is highly preferred, but we are willing to train an experienced long-term care RN with strong analytical skills.
- Skills: Exceptional attention to detail, a deep understanding of Medicare/Medicaid reimbursement systems, and proficiency with Point Click Care software.
- Mindset: A supportive team player who enjoys collaboration, communicates with empathy, and is dedicated to elevating resident quality of life.
