Licensed Practical Nurse (LPN) – Home Health
Key Responsibilities
As a Licensed Practical Nurse with BrightStar Care, you’ll provide high-quality, skilled nursing care in a one-on-one home setting, making a meaningful difference in patients’ lives every day.
- Deliver skilled nursing care in compliance with the Nurse Practice Act, BrightStar Care policies, and clinical protocols

Accurately implement RN-developed plans of care with strong clinical judgment and attention to detail

Perform all LPN-permitted skills, procedures, and treatments safely and professionally

Provide care that requires advanced LPN-level clinical skill, including medication administration, wound care, injections, disease monitoring, and specimen collection

Monitor patient conditions closely, recognize changes, and respond promptly with appropriate interventions

Maintain thorough, accurate, and timely documentation

Communicate effectively with the Director of Nursing, Nurse Managers, physicians, and interdisciplinary care team

Educate patients and families on care instructions, safety measures, and health management
-
Promote comfort, dignity, independence, and optimal outcomes for patients in their home environment
Qualifications
- Active LPN license in good standing

Minimum 1 year of nursing experience (home health preferred but not required)

Strong assessment, clinical, and documentation skills

Ability to work independently in a variety of home settings

Excellent communication skills, professionalism, and reliability

Demonstrated competence performing skilled nursing tasks within the LPN scope of practice
