We are expanding our Adult Foster Care team and are seeking a Full Time RN/ LPN Case Manager who want flexibility while still making a real impact in the community.
About the Role
This per diem/part‑time position is ideal for:
- Recent RN/LPN graduates interested in case management
- Nurses looking to transition from bedside to community-based work
- Nurses needing flexible hours to complement another position or life commitments
You will primarily work in our Adult Foster Care program with opportunities for cross-training across our other service lines as needed. You’ll blend clinical skills with advocacy, education, and coordination.
What You’ll Do
- Serve as a member-centered advocate for individuals living in the community
- Complete assessments and ongoing visits in members’ homes
- Develop, implement, and monitor individualized care plans
- Collaborate with PCPs, specialists, social workers, and caregivers
- Document accurately and efficiently in our EMR
- Participate in team meetings and quality improvement as needed
Schedule & Flexibility
- Full Time primarily weekdays
- Flexible scheduling within our operating hours (8:00am–4:30pm, Monday–Friday)
- No nights, no on‑call, and minimal weekend/holiday expectations (if any, by prior agreement)

Qualifications
- Graduate of an accredited RN or LPN program
- Current, active Massachusetts RN or LPN license
- Strong communication and documentation skills
- Comfortable using Microsoft Office (Outlook, Teams, Excel) and PDFs
- Detail-oriented, organized, and committed to person-centered care
- Valid driver’s license and reliable transportation for home visits
- Bilingual (English/Spanish) strongly preferred but not required
Compensation & Status
- Full Time hourly position
- Competitive rate based on experience
