The Resident Care Coordinator
(RCC) is responsible for ensuring the general well being of residents and clients through the
delivery of high quality services in a manner that complies with applicable state laws and
regulations. The RCC is responsible for the management and supervision of all resident care within the assisted living facility and assists in providing client care as requested by the Administrator
We are seeking a dedicated and experienced Nurse-Resident Care Coordinator (DON) to lead our nursing team and oversee resident care operations.
- Oversee the provision of high-quality care by the assigned nursing staff.
- Plans and delegates care assignments assuring effective scheduling on a 24-hour basis.
- Provides case management services to residents, ensuring the completion of pre- occupancy screening, assessment/reassessment, transfers, and health care referrals with the resident’s family, the service associates, and other necessary individuals.
- Facilitates and encourages family involvement with residents.
- Facilitates each resident’s adjustment to the facility and observes each resident’s general physical and mental condition including review of resident charts, medications, special diets and/or other orders.
- Completes prospective resident’s Pre-Screening.
- Completes resident Community Based Assessments (CBA) Individual Service Plan (ISP), Individual Evacuation Plan (IEP).
- Provides and promotes customer service for residents, employees and visitors.
- Monitors residents’ records to determine quality of service to residents and directs staff to implement needed change.
- Monitors and maintains records reflecting resident’s general condition and needs, medication review, daily record of medication administration, physician orders, logging of the drug regimen review, monthly weight, record of referrals from outside services, and record of resident incidents and accidents.
- Oversees and coordinates with the pharmacy all aspects of storage, control, destruction, and distribution of prescription and over the counter medication.
- Documents, communicates and investigates resident, staff and visitor incidents, identifies safety hazards, and institutes corrective action in a timely manner. Shares the responsibility for providing and maintaining an environment that is conducive to the safety, physical and emotional well-being of residents, staff and visitors.
- Oversees the implementation of all infection control methods and emergency procedures.
- Orders all nursing care equipment and supplies.
- Orders all medical and clinical equipment and supplies
- Participates in the quality assurance program and other committees as directed.
- Promotes wellness activities.
- Demonstrates the ability to function as a team member in sharing responsibilities for the administration of resident care in cooperation with all members of the team.
- Oversees the implementation of all infection control methods and emergency procedures.
- Develops in-service training and provides ongoing training of staff in order to provide quality care consistent with philosophy of assisted living.
- Promotes and facilitates interactive problem solving with staff.
- Responsible for scheduling all physicians services, clinics and resident appointments.
- Assists medical staff with treatment and clinic procedures.
- Provides and promotes customer service for clients, employees and visitors.
- Responsible for ensuring staff follow HR Policies and Procedures.
- Assumes responsibility in the Administrators absence.

Experience
- Must be a licensed nurse - LPN/ Registered Nurse (Missouri)
- Previous work experience with a geriatric population.
- Must possess a high degree of interpersonal relation’s skills and capability of relating to a variety of people and personalities. Considerable initiative, judgement, and leadership skills are also required.
- Must possess strong organizational and time-management skills.
- Must be able to work flexible hours.
Skills
- Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
- Must be in good general health and demonstrate emotional stability in order to cope with the mental and emotional stress of the position.
- Must be able to perform the essential physical/mental functions/requirements of the position.

Pre-Employment Screening
